care that allows for a smooth transition home from a hospital, rehab, or surgery center. Once a patient
is discharged, our caregiver will assist with the following services:
Gather his/her belongings, pack them up, and put them in the car for the trip home. (This
includes personal items and equipment.)
Ensure the patient has their discharge paperwork including discharge instructions and
medication prescriptions
Transport the patient home
Pick up immediate necessary items from store and/or pharmacy
Go through refrigerator and dispose of expired items
Provide 4 hours of in-home care services to ensure patient acclimates to being back home
